At PHC we believe that when someone is able to obtain reliable, affordable housing, they are better able to manage their own health. Our homeless outreach team and centralized intake program specialize in linking families and individuals with benefits housing resources.

SERVICES

Polk County Centralized Intake

The Centralized Intake program at PHC is a collaborative effort of more than 30 community organizations that serve people experiencing homelessness in Polk County to bring together resources to meet immediate needs in our community. Our caseworkers conduct an initial intake interview to assess needs and refer clients to programs based on need. We follow clients all the way through the referral process so that they don’t feel lost in the system.

Clothing Closet and Shower

As the home of Centralized Intake for Polk County, we have many people experiencing homelessness that come through our doors on a daily basis. We believe in providing people with a clean set of clothes and access to a shower when they are in our office. In order to best serve the people coming through our doors, we always accept donations of gently used clothes and personal hygiene products.

Below is a shortlist of things we always need:

  • Socks
  • Underwear
  • Shampoo/Soap
  • Towels

Veteran Services

 

Our homeless outreach team and Supportive Services for Veteran Families (SSVF) program specialize in linking families and individuals with the benefits, housing resources, services and care they need. The goal of the SSVF program is to improve low-income veteran families’ housing stability. SSVF provides outreach, case management services and assists participants with obtaining VA benefits as well as other public benefits that they are eligible to receive.

Eligibility requirements:

  • Must be a veteran or have a veteran in your household
  • Meet income guidelines
  • Any discharge status except for dishonorable
  • Must reside in Dallas, Jasper, Marshall, Polk, Story or Warren County
